Hours: 15 hours per week
Location: Hart District
Start date: As soon as possible
Pay: £26,188.50 FTE (Pro Rata £10,475.40)
Contract: Fixed term contract until 31/3/24 – project funding continuation anticipated
Our standard terms and conditions include:-
37.5 hours is an FTE working week 09:00 to 17:00 with a 30-minute lunch break
25 days holiday plus bank holidays
If eligible, 5% employer pension contribution to Peoples Pension workplace pension scheme with a 3% minimum employee contribution
We are looking for an adviser to provide advice to vulnerable patients on a specified range of subjects, primarily related to addressing Fuel Poverty and Debt prevention – as wrap-around CA support.
Home and Well is a unique collaboration of organisations working together to support residents of Hampshire and the Isle of Wight to return home as quickly and as easily as possible after a hospital stay and, importantly, ensure they have the comfortable home environment that will help them continue to recover rather than relapsing.
The project combines all the elements of helping clients with fuel/water poverty and identifying PSR cases, whilst also providing a more holistic approach and signposting clients to services helping to tackle additional issues such as housing, employment and relationships.
